Based on information gained at assessment, each tenant works out an Individual Support Plan with her support worker. The aim of the Support Plan is to provide a sense of direction and purpose for both the tenant and support worker. Support Plans are unique to each tenant as they are about her needs and goals. Support Plans cover a range of issues such as: personal care and safety, health, money matters, personal development and employment.
Each tenant can expect her support worker to:
- provide emotional support (not counselling)
- advocate on tenants behalf
- speak with and make referrals to other agencies
- provide information about what is available within the community
- be informed about relevant Government services and procedures
- assist with locating and securing long term housing
